Kevin got his wings Thursday August 9, 2001. His cancer did NOT survive, but Kevin's spirit will live on forever. Kevin went to be with God peacefully and with a smile. We love you Kevin.

There is a $15.00 Pre-registration Entry Fee (which includes a T-Shirt). One entry form is required per participant. You may mail in this form and pay the fee with a check or money order. Additionally, you may pay online with a credit car (via PayPal.)

When you have filled in the form you may print it. When you click on the SUBMIT button you will be taken to a page that will allow you to choose a method of payment.
